What is a Home Care Package?

At Supreme Care we completely understand and respect the desire to remain at home. However, you or your loved ones may require some assistance in the areas of cleaning, meal preparation, gardening, transport and shopping etc. There is a range of approved Home Care Packages that Supreme Care can put together for you.

The Government’s My Aged Care packages are made up of four levels, designed to give you the care you might need now but then can also be adjusted if your needs change in the future. Home Care Levels 1 and 2 help people with basic or low-level care needs; Levels 3 and 4 help people with intermediate to high care needs. Alternatively, if you are a privately funded client, we can tailor a customised Home Care Package based on your needs.

Our full range of  home care services are flexible and can be tailored to suit your changing needs. Our comforting and helpful staff will accommodate any length of time needed. Whether your situation calls for short term home assistance or an on-going span of care, we’ll be there when ever you need us. Supreme Care services are flexible and “one off” or emergency services can be arranged at short notice.

WE PROVIDE  FOUR HOME CARE PACKAGE LEVEL OF SUPPORT:

  • Leve 1 – for people with basic care needs
  • Level 2 – for people with low-level care needs
  • Level 3 – for people with intermediate care needs
  • Level 4 – for people with high-level care needs.

Our Services

    The services we offer under these programs can include:

    • Personal services: assistance with personal activities such as bathing, showering, toileting, dressing and undressing, mobility and communication.
    • Social Support: Assists people to participate in community life and develop social connections through group-based activities.
    • Nutrition, hydration, meal preparation and diet: assistance with preparing meals, including special diets for health, religious, cultural or other reasons; assistance with using eating utensils and assistance with feeding.
    • Continence management: assistance in using continence aids and appliances such as disposable pads and absorbent aids, commode chairs, bedpans and urinals, catheter and urinary drainage appliances.
    • Mobility and dexterity: with an Occupational Therapist (OT) referral crutches, quadruped walkers, walking frames, walking sticks, mechanical devices for lifting, bed rails, slide sheets, sheepskins, tri-pillows, pressure-relieving mattresses and assistance with the use of these aids, can be provided.(Case Management fees may apply please contact us for more information)
    • Nursing, allied health and other clinical services: with a referral for speech therapy, dietitian, podiatry, occupational or physiotherapy services, support to hearing and vision services and access to other specialist health and well being services can be coordinated as required (Case Management fees may apply please contact us for information)
    • Management of skin integrity: assistance with skin emollients.
    • Home Maintenance:maintain the home and garden in a condition and of functional safety and provide an adequate level of security.
    • Domestic services: house cleaning.
    • Personal Laundry service: Includes laundering of client’s clothing and bedding that can be machine washed and ironing, arranging for dry-cleaning of client’s clothing and bedding that cannot be machine washed.
    • Home risk assessment: conducted by the Care Coordinator to ensure client and staff safety before commencing services.
    • Support for clients with cognitive impairment: coordinate activities and access to specific programs designed to prevent or manage a particular condition or behaviour, enhance quality of life and provide ongoing support.
    • Call in services:available for a chat anytime with our friendly staff.
    • Transport and personal assistance:assistance with shopping, visiting health practitioners and attending social activities
    • Sit in service: companionship while family members run errands for short periods of  

    • Modifications to the home: Coordinate referral to the Occupational Therapist, to assess and assist with easy taps, shower hose or bath rails installation (Case Management fees may apply please contact us for more information)
    • Rehabilitative support: Coordinate allied health referral or providing help to access rehabilitative support, to meet a professionally determined therapeutic need (Case Management fees may apply please contact us for more information )
    • Emotional support: including ongoing support adjusting to a lifestyle involving increased dependency and assistance for the client and carer if appropriate

     

    Hourly Home Care

    We provide complete flexibility with your services and can visit for as little as one hour at a time. You are welcome to call us even with little notice – we can match a caregiver quite quickly if required.
    We can book regular services each week to give you peace of mind that your support is regular and consistent.
     

    24 hour Care

    When recovering from an injury or illness or after being diagnosed with a chronic health condition, live-in care is a beneficial option for many seniors. This level of service provides constant assistance and supervision at home and the highest level of support for recovery and disease management. Live-in care also has the added benefit of offering companionship for housebound seniors. At Supreme Care, we pride ourselves in providing the highest-quality home care experience. Our live-in caregivers make the home safer and more comfortable for ageing seniors by empowering them to maintain independence and a higher quality of life. 

    We’ve put together a step-by-step guide to apply for home care package funding through the My Aged Care programme:

     

    STEP 1  Give Supreme Care team a call

    With  experience dealing with the Government’s My Aged Care packages, we’d be more than happy to assist you.     STEP 2  Get in contact with My Aged Care

    When you’re ready you will then need to get in touch with the My Aged Care contact centre where you can request a Home Care Package on 1800 200 422 or visit www.myagedcare.gov.au. During this process, they will confirm that you meet all the government requirements for aged care funding. Don’t forget, when you call My Aged Care, be sure to have your Medicare card close by and if you are calling on behalf of someone else, make sure you get their consent.

     

    STEP 3 An ACAT assessment will take place

    An ACAT assessor, who is an experienced clinician, will come and visit you at home. It’s here where they will discuss with you your personal circumstances to assess and workout what the right level of support might be needed.

     

    STEP 4  Get your results and jump in the queue

    Once your assessment is complete, you will receive a letter of to let you know whether you are eligible and approved to receive a Home Care Package. Then you will then be placed in a national priority queue and be contacted when a suitable package becomes available. Please note, that your position in this queue is based off how long you’ve been waiting for care as well as your personal needs and circumstances. It is also important to consider that there may be costs involved and you may need to complete an Assets and Income Assessment.     STEP 5  Receive your Package and Start Services

    When you’re ready to commence your care package, you will receive a letter from My Aged Care confirming everything and providing you with a unique referral code. Once you receive this code, please get in contact with Supreme Care team and we can start to plan and organise your personal Home Care Package.
